MDS (Minimum Data Set)

The LifeCare Plus MDS (Minimum Data Set) application is designed to streamline the generation of health center resident assessments and reports. MDS can be used as a stand-alone clinical system or in concert with other LifeCare Plus modules for facility-wide automation. The application is compliant with all state and Federal regulatory guidelines, and has numerous features geared toward helping care providers achieve HIPAA compliance.

• Users view and enter data in a Windows GUI (Graphical User Interface) environment with the Minimum Data Set forms displayed just like the standard CMS MDS forms
• Automatically fills in most items in MDS sections AA, AB, and A with data input through the admissions information screen
• Multi-user and multi-facility capable
• RAPS for the MDS assessments are triggered automatically
• Case Mix RUG III scores are calculated automatically for use with the Medicare Prospective Payment System (PPS) and for Medicaid in states that use the case mix scores
• Automatically performs extensive error checks on the MDS assessments
• Users can print the MDS assessments exactly as shown on the screen, and print blank MDS forms if necessary
• Automatically calculates Resident Census and Conditions of Residents, and allows users to manually make changes to the report
• MDS cross-reference lists can be displayed, modified, and printed
• Automatically calculates Resident Roster Matrix and allows users to manually make changes to the report
• Users can print or display the Resident Roster Matrix by resident name, ID, or room number

  • Admission List Summary Report - Summary report of admission information
  • Admission List Detailed Report - Detailed report of admission information
  • Assessment Due List Report - Lists when the next quarterly reviews and annual assessments are required, and when they're past due
  • Assessment History List Report - Lists residents' current and past MDS assessments with descriptions
  • Incomplete List - Lists any incomplete information on the MDS assessments
  • RAPS Report (Section V) - Prints out the RAP Summary (Section V) of the selected MDS assessments, guidelines and trigger legend
  • RUGS III Calculation Report - Lists the RUG data for the proper MDS assessments
  • Error Log Report - Lists the current errors on the MDS assessments
• Automatically calculates RAP Modules for MDS assessments including what items triggered the RAP
• Allows for data imports from any other system using a CMS MDS 2.0 submission file with no re-keying required
